Dr. Martin Schwarz - 5 questions for the shoulder specialist

Why did you decide to specialize in orthopaedics and trauma surgery?

The subject of trauma surgery, now called orthopaedics and traumatology, fascinated me right from the start because of the need for considered, rapid action with visible benefits for the patient. Even as a young student, I enjoyed working in the operating theater, in the outpatient clinic, in the trauma room and on the ward so much that it was very important to me to learn this subject.

Through the support of experienced colleagues at home and abroad, I was able to gain experience and develop myself further.

What fascinates you about the shoulder?

The shoulder with its tendons, ligaments and muscles is a challenging joint due to its great freedom of movement.

The treatment options have changed so much for the better over the last 30 years that the fascination of maintaining these degrees of freedom prevails.

Which shoulder complaints are most common in your daily practice?

Impingement complaints dominate in daily practice, whether due to calcification or simply non-specific pain. Torn tendons and instabilities of the shoulder joint come in second place in my practice.

Which conservative therapies work best for shoulder pain?

The conservative treatment options of infiltration can lead to long-term pain relief in some cases, but new treatment options with the body's own growth factors and in combination with shock wave treatment promise a lot, especially in terms of lasting pain relief.

What should patients pay particular attention to after shoulder surgery?

After shoulder surgery, you will receive detailed instructions on what to look out for.

The special features depend on the operation performed. A tendon suture has different rules of conduct than an osteosynthesis or stabilization operation.

My most important career milestones:

  • On January 1, 2021, I started my career in private practice. I carry out my surgical work at the Vienna Private Clinic and other Viennese hospitals. This means that my experience is still available to my patients around the clock, as usual. Thanks to my contract with the BVAEB and KFA, these operations can also be carried out as a “health insurance benefit”.
  • The emergency service at the Vienna Private Clinic ensures that you are treated by me in acute cases and that further follow-up treatment is organized in my surgery. Further access to the rehabilitation facilities of our healthcare system is of course open to you postoperatively.
  • From 2006 to 2020, I was head of the trauma surgery department at the Mistelbach-Gänserndorf Regional Hospital, a key hospital in the Lower Austrian Regional Hospital Holding. We treated 60,000 freshly injured patients per year and performed 2,800 operations, whereby, in addition to my organizational activities, special emphasis was placed on the surgical treatment of complex cases of the pelvis, spine and shoulder joint. That means routine.
  • I was able to carry out and improve my specialization in “shoulder surgery” in 2000 and 2001 at the Trauma Surgery Department at the Salzburg State Hospitals (now Paracelsus Private University).
  • I learned the specialty of trauma surgery, with everything that goes with it (the severely injured patient), at the Second University Clinic for Trauma Surgery at Vienna General Hospital and then at the Trauma Surgery and Sports Traumatology Department of the Danube Hospital in the Social Medicine Center East (SMZ-Ost), where I perfected my skills over a period of 13 years. Here I learned and experienced that swift medical action and careful work belong together and that this is the only way to reintegrate a severely injured patient into their profession and social life.
  • The focus on the shoulder has been with me for over 25 years. Many methods have been used during this time and some have been abandoned. What has remained is the “overall view” of this fascinating joint, which is the most mobile in the human body.
  • The function of the elbow and the hand depend on a functional shoulder. While the rest of the world separates open shoulder surgery from arthroscopic treatment, I have made the best of both worlds my own. This means that all surgical methods of shoulder surgery are used and mastered. From osteosynthesis to endoprosthesis or from tendon suture to tendon reconstruction by means of tendon transfer - many things are possible.
  • In times of increasing osteoporosis, reconstructions of bone fractures of the humeral head, shoulder girdle and upper arm are essential in order to restore the entire limb to working order after a trauma. Fractures of the humeral head in particular represent a special situation. It is still widely believed that conservative treatment is sufficient. However, it has been shown that the mobility of the shoulder suffers as a result of the necessary immobilization period. Too short a period of immobilization, however, leads to the formation of a false joint with subsequent severe loss of function. The same applies to fractures of the humeral shaft.
  • Only a stable reconstruction and early functional rehabilitation can prevent a long period of suffering.
  • Focusing on the individual - that is my credo, the right treatment in the right place at the right time.
